A static caravan is a great investment in your families future holidays. You can enjoy amazing times in your new holiday home but you should look at after your static caravan to ensure it stays in tip top condition year on year.

I thought I would give you some hints and tips on maintaining your caravan and I will write some more detailed advice in other articles, but this one covers the main areas.

1) Gas and Electric Maintenance

Your holiday home will most likely be fitted with a gas boiler which in some cases will be connected to a central heating system.

Annual maintenance of your boiler should be completed by a competent and registered engineer, however you can ensure that it’s running smoothly and highlight any concerns to your engineer as soon as possible. This will keep your overall costs down in future years.

As for electric systems I would recommend getting these annually checked for safety. Again, you can check if the fuses are in place correctly and that when you leave your static caravan that both the electric and bass is turned off at the main connection points to your plot.

2) Exterior Maintenance

Dirt will collect on your caravan throughout the year. Cleaning the exterior of your caravan will keep it looking great and will also ensure it stays waterproof.

You can use a jet wash or a cloth and detergent to clean the outside of your caravan. Windows can be cleaned with a normal window cleaning solution.

Ask your park manager if they offer a cleaning service or know someone who does if you feel this is a too greater task for you.

3) Guttering

One area to check on a regular basis are your gutters.

Remove any leaves and debris to ensure that water can pass easily away. This is a common cause of damp in caravans so by performing this quick and simple maintenance task your static caravan will perform as expected for many years.

4) Anchor your static caravan

You might think that your caravan weighs quite a bit, but high winds in the autumn and winter can cause them to flip over.

5) Window Latches and Doors

A new caravan can be installed with double glazing and uPVC doors. However I would advise that all windows and doors close and lock securely.

A static caravan can warp as it gets older, so checking that the seals are tight and all security features are in good working is a quick precaution that can save you money in the long term.

6) Check for rust

The chassis of your caravan will be constructed from a hardy metal however you should check for rust on other areas as your caravan gets older. This would especially be the case on any fittings attached to the caravan.

7) Wheels

It’s rare to have to move your static caravan however an annual inspection of your wheels will ensure that if it is required to be moved it will easily without issues such as getting cranes in.

8) Electric Fridge and Freezer

About one in four people admitted in a hospital with depression, are there because of psychotic outbreak.

What is psychotic outbreak, then?

Basically, this is a state where you detach from reality. Now researchers say, that people who take the pill improve dramatically within days, and what’s more important the effect lasts, which means that they can get better without suffering effects of psychosis for months or going through traumatic electroconvulsive therapy.

Nutrients for Bone Support

The simplest way is to put a color to an item. A red apple, a black cat, a blue fish, etc. These all create a more specific and detailed vision to the reader. But they are very common and simple adjectives. There is more than just red, orange, yellow, green, blue, purple, brown, white and black. There is a whole spectrum of thousands of colors. Having a wider vocabulary when you want to be specific can not only help the reader's vision and the story's imagery, but it also increases the chances that the reader will not get bored as quickly.

There is more than just picking a color though. There is comparison. You can use another well known object and its color to help strengthen the imagery. For example:

The leaves were red and oranges. vs. The leaves were colors of flickering fire.

The shirt was pink. vs. The shirt was pink like fresh rose petals ..

The wine was purple. vs. The wine swirled with colors of amethysts and plums.

Both ways can be very effective. It's all about emphasizing certain things with important detail. You do not need to tell the reader what color everything is. But being able to effective paint a picture of words with color makes the reading clearer and more enjoyable.

Do not be afraid to take chances. Try new things when describing detail. During editing or reviews from others you can decide if it is not enough or too much, or hopefully just right. Things may sound overdone at first or awkward, but they can be fixed easily. The important thing is to have fun. The piece will speak for itself once you really get into it. Just go with the flow and enjoy creating interesting and unique imagery with colors.

Learn the Six Levels of a Dog-Bite

Aggression is a survival skill required by all canines. Dogs bite for many reasons. Over the past two decades it has become “trendy” amongst dog trainers and behaviorists to attempt to resolve and eliminate dog aggression. A typical definition of “bite” is “to seize with the teeth so that they enter, grip or wound.”

To quantify the degree of severity, Dr. Ian Dunbar, has created the following chart/guide on bite “levels”.

Level 1: This bite does not touch the skin. The dog is air biting or snapping.

Level 2: This bite makes contact with the skin, but doesn’t break the skin. Pain and bruising may result, but no abrasions will be visible.

Level 3: This bite ranges from a one to three punctures in a single bite with one puncture less than ½ the depth of the eye-tooth (fang) with or without some tearing.

Level 4: The dog is putting great pressure into the bite. 1 to 4 puncture wounds with or without tearing, more than ½ the depth of the eye tooth. This is usually accompanied with bruising and likely to require medical attention. These injuries suggest the dog grabbed and shook what was in its mouth.

Level 5: Multiple level 4 bites. This dog is usually beyond the ability to reason and may feel his/her

life is threatened.

Level 6: The dog has killed.

Biting can also result when the dog is highly aroused/excited when chasing people along a fence line or from a tie out/chain. This is called “barrier frustration” and it can (and does) occur commonly though most dogs don’t bite. Bites usually occur when the dog’s owner/handler attempts to physically control the dog. The dog is so aroused, that he indiscriminately bites the person. This is called “displacement aggression” and is quite common.

Most large powerful breeds can cause more severe injuries than small dogs. Keep in mind that it is mixed breeds and not pure bred dogs are the type of dog most often involved in inflicting bites to people.

Some warning signs of aggression and biting may be possible:

o Any health change or long term health problem

o Anxious or hyper behavior (fear of new or certain people or places)

o Biting or a history of biting (past behavior is a predictor or future behavior)

o Growling

o New behavioral changes (sudden obstinance)

o Obsessive fence/kennel running/chasing

o Severe separation issues

o Uncontrollable lunging at people or dogs while on leash

Most dogs that inflict Level 6 and level 5 bites are euthanized. Level 3 and 4 biters need serious behavior modification to remove any further risk. Level 1 and 2 biters should also be helped with relative ease and the guidance of a behavior consultant.

However, if your dog is obedience trained, maintains a social life (regularly gets out of the house & yard), is neutered or spayed, healthy and is female, your dog is less likely to bite. But, that doesn’t mean he won’t. After all, anything with teeth can bite!

We none know what we are doing. Though we go about our days with full intent, saturated purpose and determined objectives, the real sublime reason why we are motivated to act remains hidden from view. We awake in the morning with a concentrated objective, having calculated the outcome and minimized the barriers; we proceed as if what we will achieve has something to do with us. So much so, we either take blame for the mishaps or bask in the glory of the success. Neither response is accurate or appropriate. “IF at any time, we think we know what we are doing, cease immediately.” In other words, we never know; we only project and in those projections, we are wrong.

There’s much more to the picture left unseen, than seen.

So much is hidden from view. It’s simply amazing how we go about our days denying how much crucial information we don’t have access to. We work with what with have in an obscured partial light; though, we attack the problems as if we have all the goods available. The truth is our present knowledge and insight {no matter how much we’ve paid for the expert guidance} is limited and biased.

Yet, we continue to make all of these unnecessary plans and believe that unless they unfold exactly and precisely as we have ordered (constructed), we have in some way failed to accomplish our preset goals. Needless to say, when it appears as if our specific goals have not been met, we have in no way failed. On the same token, when it appears as if what we set out to do was fully satisfied, the end is not in sight, either.

‘Nothing’ is at it appears to be. And, we have very little to say or do about whatever occurs.

But, ‘say’ we do! That falls under the category of tenacious gossip.

There is a subtle esoteric law of inference that is applied to each and every act we undertake: purity of purpose. It’s totally impersonal and some may say, irrational, but none the less effectual. Beyond our physical ability, mind’s intellect, and emotional barrage of thwarted desires (enacted to interfere), a governing hallowed force guides and directs all of our actions. We are not consulted on the matter, nor are we given the privilege to decide if the outcome is needed or not. These sacred acts are decreed from a much higher sphere of authority which bypass the normal functionability of man. Does this mean all of our actions are predestined? In a vehemently resisted word: yes. {Look at it more like we are prodded like cattle into a specific sphere of pre-chosen activity of earth experience.}

We were born with an explicit destiny, a sublimely definitive work to accomplish while on earth. We will achieve that definite purpose. Whether or not, we ever see the picture clear enough to appreciatively discern our personal effort’s output, remains irrelevant. “Purity of Purpose’ determines the result of every single action we administer. IF it falls out of line with that celestially projected intent, it’s simply not going to happen. Now, we may wonder (humans continually question everything; it’s part of our curious nature), if will we be able to discern when we have ‘Purity of Purpose’.

In a definitively resisted word: no. IF we could see ‘IT’, it wouldn’t be ‘IT’.

In addition to the ‘curiosity seeking creature’s mind’ we inherently imbue, we are just as impatient, IF not more so. And, IF we were left to our own gluttonous appetites of unrestrained pleasure, to indulge in so many variant earth experiences, we would certainly move prematurely into areas not necessarily suitable for our evolvement and soul’s preference. Oh. Did I not mention IT is our Precious Ruthless Soul which has the definitive say about what will be and what will not be?

Again, a matter of Semantics, the term ‘SOUL’ can be viewed, argued, and defined in many different philosophical, scientific, religious, ethical, cultural, environmental, psychological, biological and mental constructs.

However, for brevity of purpose: ‘much more remains unseen than seen’.

IF, in fact, we are on earth to make some sort of evolutionary progress (again, we can’t gauge it), we would only be able to do that by and through isolated (personal) incidences where we had no way or knowing, explaining, defending or excusing what took place or did not occur. In other words, it would be in those indescribable times, when and where, we were unable to logically deduce any practical means (commonsense) for what had occurred and were simultaneously driven to find either resolution or solace from within that the inexplicable becomes acceptable.

IF, in fact, we are all on some sort of spiritual journey, the tolerable foundation of our ‘inches in steps’ to our so-called pre-set established directions become vitally necessary discipline. The deal for real is plain and simple. MASTERY. We are working on ourselves, in as much, as we are learning to surrender to a much higher force of operation. {Who incidentally remains privy to much more elaborate information than we are capable of realizing.}

In no way, am I attempting to personify this ineffable energy, I simply am using the restricted barriers of language to convey a point. Through a laborious solitary journey, the seeker (human being) learns as many techniques, practices and skills (much like an adept musician) in order to finely tune, manipulate, orchestrate and demonstrate his enriched abilities. Not without countless hours of careful observation, dedication and consecration of committed study to bring about a much deeper understanding and comprehension of the higher self in operation while alive.

To understand that we are not to look outward for the manifestation of results as a sign of our progress but to remain firmly rooted turning away from the distracting voices of either praise or condemnation (filled with fear, regret, remorse and anticipation) to the ‘still silent small voice’ of knowing everything is exactly and precisely as it should be for reasons we can’t see propels us to move, breathe and have our being.

Purity of Purpose stimulates us to experience without any trace of a lust of result. Therein lies the secret of man’s esoteric enhancement without a coveted design.

Reverse Mortgages – What Does the Term Principal Limit Mean?

When explaining a reverse mortgage to a senior homeowner, one of the most important terms a reverse mortgage loan officer will discuss is the “Principal Limit.”

What is the Principal Limit and why is it important?

The Principal Limit (PL) is the gross amount of money the lender is willing to lend to the borrower of a Home Equity Conversion Mortgage or HECM reverse mortgage, based on a formula derived from Congressional legislation and implemented by the Department of Housing and Urban Development (HUD) and using the following three criteria: 

  • The lower of the Maximum Claim Limit or the Federal Housing Administration (FHA) appraised value of the home;
  • The age of the youngest borrower (must be 62 or older);
  • The current expected interest rate (based on the current 10 year London Interbank Offered Rate, or LIBOR rate, plus a stated margin for the adjustable rate HECM and based on the current fixed interest rate for the fixed rate reverse mortgage).

The three listed criteria affect the PL in the following ways:

  • The higher the value of the home (up to the maximum claim limit of $625,500) the higher the amount of the PL will be;
  • The older the youngest borrower (age is always based on the youngest borrower’s age, not a blending of multiple borrowers’ ages) the higher the amount of the PL will be;
  • And, conversely, the higher the current expected interest rate, the lower the amount of the PL will be.

The reason potential borrowers should become familiar with the term Principal Limit and what it means is because it is from this cash figure that all fees and set asides will be subtracted in order to arrive at the maximum cash or loan proceeds available to the borrower.

Congress Plans to Lower the Principal Limit

Congress lowered the Principal Limit for the fiscal year 2010 signifantly to make up for a perceived budget shortfall of approximately $798 million for HECM reverse mortgages put in place within that fiscal year. HUD has announced that for the fiscal year 2011 there will likely be decreases in the Principal Limit as well. The 2011 year begins in October 2010 for budgetary purposes.

 Until the budget bill has made it through the joint Senate and House committee, been voted on and signed, we do not know what the exact amount of the cut in the principal limit will be. Senior homeowners who have investigated HECM reverse mortgages prior to October 1, 2010 should contact a reverse mortgage lender to learn how the decreases in Principal Limits could impact  them personally if they pursue a reverse mortgage.

Golf Swing Tips: Avoid Straightening Your Left Leg Until After Your Club Face Contacts Your Ball

One of the best golf swing tips is to keep your left knee flexed until after your clubface has contacted your ball at impact.

The reason why this is one of the best golf swing tips to improve your golf game is that if you straighten your left leg and lock your left knee joint before impact, you will mishit your golf shot.

Specifically, locking your left knee joint before your clubface contacts your ball during your down swing will change the bottom-out point of your swing arc and prevent you from hitting straight golf shots.

Even if you setup perfectly to your ball and otherwise swing your club to hit a golf ball straight, if you lock your left knee joint as your clubface contacts your ball during your down swing, your ball is going to fly anywhere but on a straight path to your target.

Some of the leading golf instructors imply you should straighten your left leg as your clubface enters the impact zone of your down swing.

They don’t specifically state you should straighten your left leg and lock your left knee joint at the point of impact, but if you listen to some of them, you may be led to believe such.

For example, some instructors and commentators imply you should straighten your left leg when they talk about the importance of hitting against a firm left side. A few even suggest you should straighten your left leg at the point you deliver your clubface to your ball during your down swing.

Such suggestion definitely is not in keeping with the bestgolf swing instructions. However, I do not believe most of those who imply you should straighten your left leg at impact truly mean to suggest such an absurd idea.

Instead, I just believe they get caught up in the mechanics that you must hit against a firm left side. They then mislead golfers into assuming they should straighten their left leg during the impact zone of their down swing. My take is they really mean to suggest that you should straighten your left leg immediately after impact, if at all, not at impact.

Irrespective of their intent, I often observe golfers on a practice range locking their left knee joint as they deliver their clubface to their ball at impact. As a result, they mishit every golf shot.

Therefore, regardless of what you may have interpreted from listening to golf instructors and commentators about hitting against a firm left side, please do not conclude that they mean to imply you should lock your left knee joint as you deliver your clubface to your ball at impact.

For those of you who have misinterpreted such comments from golf instructors and mistakenly adopted such advice into your golf swing, hopefully this article will clear up any such confusion and misinformation.

Otherwise be prepared for a disaster if you habitually lock your left knee joint before your club face contacts your ball

How to Play Violin Chords – Which Asks the Age-Old Question "Are There Chords on the Violin?"

I’ve been a guitar player since I was 14 years old, so chords have always been the backbone of my playing knowledge. I was interested in broadening my musical horizons and picked up a violin at a pawn shop for $180.00, a stradivarius copy, but a nice violin with case and bow. It was broken on the inside, so I took it to my brother, the only violin player in our family, and asked him to fix it. It was well worth the $180.00 I spent after he repaired it.

Now came the fun part…learning. Imagine my surprise at picking up my new violin and, for one thing, there were no frets! This was challenging, and yet…how was I going to play it? I could have asked my brother, but there are 1350 miles separating us, so that proved to be difficult.

I spent some time doing some research about how to play violin chords. There was not as much information available as I thought there would be, but there was some.

Here is what I learned:

The violin is mainly used to play continuous notes by drawing the bow across the strings in a continuous motion. You can create combination notes by changing up your fingers on the fretboard to create “slurs”. These are notes that are to be played without separation.

The chords that have been used on the violin are actually the ones for the mandolin, another instrument tuned to the G, D, A, E sequence, but played with a pick. Somewhere in history, someone experimented while playing the violin and used the mandolin chords to create music.

What a great opportunity this was to use the violin in a different venue, to create a different sound entirely!

These chords are simple fretting which only requires the holding down of 2 strings and strumming of all or only a couple. What a fascinating variety of sound you can achieve by changing things up a bit.

So, in essence, to play an A “chord”, you hold down the D string on the second fret area and strum both the A and D strings.

The only problem that arises is for the new violinist who is not familiar with his fretboard. Without knowing what notes are actually played on the fretboard, the chording patterns will be useless to the new violin player.

So, it makes sense to learn the fretboard before you try to learn how to play violin chords.

Plugged Milk Duct – 4 Tips to Unclog It

Got plugged milk duct? Are your milk ducts blocked, inflamed and / or sore. The first general symptom is the presence of a small hard lump. This may be sore when touched, or may be a very tender spot in the breast.

Clogged ducts may be as a result of failure to drain milk completely. Tight nursing bras, incomplete or skipped feedings, insufficient expression of the milk, an illness or stress are all factors that contribute to the development of a clogged milk duct. Here are four important tips in preventing clogged milk ducts.

Tip 1

You should nurse your baby, pump your breast with the use of your hand or breast pump more frequently. Removing the milk prevents engorgement and infection. Nursing should be connected frequently and for long periods to ensure the baby removes enough milk from the breast. Latching the baby well drains the breast more adequately and you should place the baby's chin towards the affected area.

Tip 2

Clogged ducts feel like a small lump and should be massed with a pushing motion towards the nipple while nursing your baby or pumping. A heating pad or a warm moist towel can also be placed on the area that sees to be clogged and repeated as often as needed. The heat opens the milk ducts better and allows for more milk drainage. A hot shower or bath can also promote relaxation and unclog the milk duct. Cabbage leaf warmed in the microwave can also be used as a home remedy and put on the affected area. Hand held massagers and reusable heat packs can also be used until the duct becomes loose.

Tip 3
Milk clogs can be squeezed out of this breast. This should always be induced under sterile and hygienic conditions to prevent infections. The breast is massaged until white clot appears on your nipple. Moist heat should then be applied and a sterile needle is then used to pop the blister. An antibiotic ointment should then be administered for a couple of days to prevent infections from occurring. Avoid excessive arm exercises at the gym if you are likely to get clogged milk ducts. Persistent clots with other symptoms such as feeling aches, fatigued or feverish may be a sign an infected duct. A doctor should be consulted when this occurs.

Tip 4

To relive a clogged milk duct, mothers should nurse with the breastfeeding hanging straight down. Place the baby on a bed and kneel over him to nurse. Leaning as far over as you can while dumping, can also aid in the removal of the clog. You should not wear tight fitting clothing or bras. This pressure can cause a milk duct to clog. Bras that contain underwire should also be avoided as the wire can cut into the breast and cause an infection. Supportive, not tight bras can alleviate some problems associated with clogged ducts and reduce the amount of clogs present.

Bandages and Dressings for Hidradenitis Suppuritiva

Hidradenitis occurs when a plug of dead skin in the duct of a gland emptying into a hair follicle in areas of opposing skin surfaces like the armpits, under the breasts and in the groin initiates an infectious process as bacteria multiply within the plugged gland creating a skin boil. The obstructed gland or boil ruptures into the deep layers of the skin; adjacent glands become involved; and abscesses form. Subsequently, multiple draining sinuses or holes appear on the skin surface and the whole hair bearing area may become inflamed. We call this process Hidradenitis Suppuritiva (HS).

As I described my first choice of treatment for this process is surgical removal of the involved skin and closure of the resulting wound with a flap of adjacent normal skin. However, this aggressive surgery is not always the best near term option because of insurance, financial, work or personal reasons. For early stages of the disease temporizing the situation with topical treatments is appropriate and can provide significant relief.

The Hurley stages of the disease are as follows:

  1. Solitary or multiple isolated abscess formation without scarring or sinus tracts. (A few minor sites with rare inflammation; may be mistaken for acne.)
  2. Recurrent abscesses, single or multiple widely separated lesions, with sinus tract formation. (Frequent inflammation restrict movement and may require minor surgery such as incision and drainage.)
  3. Diffuse or broad involvement across a regional area with multiple interconnected sinus tracts and abscesses. (Inflammation of sites to the size of golf balls, or sometimes baseballs; scarring develops, including subcutaneous tracts of infection. Obviously, patients at this stage may be unable to function.)

We now know that there is an inflammatory process present in hidradenitis before there is any sign of infection such as an abscess and I call that pre-stage I. In that stage the skin is intact with redness, tenderness, warmth and burning pains just before an abscess appears. In this early stage there is no drainage and a raised swelling may form a nodule which is very different from an abscess. Some doctors prefer antibiotics and injections of corticosteroids into the affected areas to treat early Stage 1 disease. Others give larger doses of steroids by mouth or testosterone antagonists. Stage III is not amenable to these temporizing measures because of the degree and depth of skin damage but these measures can delay disease progression when employed early in stage I.

Staging of the disease and its appropriate treatment is complicated by a tendency of the disease to go through quiescent and active phases. The person afflicted with HS applies a medication, swallows a pill or changes their diet in some way at a time when the process is about to go quiescent and then mistakenly attributes the quiescence to the medication, pill etc. The treatments described here have been vigorously studied by numerous groups on a variety of wounds including HS. They are employed at assisted living facilities and nursing homes across the country each day.

The topical treatments can be divided into types of dressings and medications either applied directly to a wound or impregnated into the dressing.

A dressing is something such as gauze that is applied directly to a wound to promote healing, stem bleeding, absorb wound drainage, remove dead surface tissue, prevent medications applied to the wound surface from evaporating or falling off and protect from or treat infection. In medieval times they were applied mostly to prevent others from seeing ones wounds. Historical belief that a wound should be kept dry in order to heal faster without infection were dispelled in the 1960s when studies showed that wound surface cells died when they were allowed to dry out thereby prolonging the time required for healing. In the last 20 to 30 years a vast number of different types of dressings with different properties or functions have been introduced.

The TYPES OF DRESSINGS can be grouped as follows:

–ALGINATE DRESSINGS

are made of seaweed alginate and expand as they turn from a gauze like consistency into a gel consistency while absorbing drainage from a wound

–CONTACT LAYER DRESSINGS

are made of low or non-adherent material that allows drainage to pass through the material

–FOAM DRESSINGS

are made of material that absorbs drainage and actually sucks fluid out of wounds

–HYDROCOLLOID DRESSINGS

prevent leakage from a wound and keep the wound surface moist while preventing outside moisture from reaching the wound. They look like thin pieces of pliable plastic that come in a variety of thicknesses and shapes and stick directly to the skin around a wound. The most common brand used is Duoderm.

–TRANSPARENT DRESSINGS

are waterproof keeping outside moisture and bacteria away from a wound while keeping the wound moist and easy to evaluate as you can see through them. The most common brand used is Tagaderm.

–MESALT DRESSINGS

are impregnated with sodium chloride. They clean up the wound a bit by moistening devitalized tissue and are also used for moderate draining wounds especially those with tunneling or undermining

Once you have picked a dressing the next choice is whether or not to apply medication on the wound and under the dressing or impregnated into the dressing. Dressings impregnated with medications from the manufacturer can be very expensive in which case you can save significant money by applying the medication to the dressing rubbing it over the surface of the wound and then taping the dressing in position. Unfortunately that is not possible with silver.

The AVAILABLE MEDICATIONS include:

–HYDROGEL

are mostly water in a gel base and are applied to dry wounds to keep them moist and prevent wounds from drying out

–CLINDAMYCIN OINTMENT

In 1983 a double blind study found that topical clindamycin antibiotic performed better than a placebo in diminishing abscesses, inflammatory nodules and pustules of hidradenitis in the first month of treatment with less of a difference between the 2 groups after 2 and 3 months of treatment. No side effects were recorded and the treatment was easy to administer. Topical clindamycin is best used in preparing patients for surgery so there is less chance of a complication after surgery.

–RESORCINOL

is an antiseptic and disinfectant that is the active ingredient in over the counter acne medications like Clearasil. In a 2010 study, topical treatment with 15% resorcinol reduced pain from painful nodules in all patients with hidradenitis suppurativa. The medication cannot be used over areas that have lost skin integrity because it can be absorbed into the circulation and cause a blood disease called methemoglobinemia (resorcinol poisoning). Drainage is a sign of lost skin integrity and therefore precludes the use of resorcinol.

–MEDIHONEY

References and formulas for honey-based wound dressings can be found throughout the medical writings of ancient Egypt, Greece, Rome, India, and China. Honey was used as a first line treatment until the mid-1900s when synthetic dressing materials and antiseptics grew in popularity. Multiple studies going back to 2004 show antimicrobial, anti-inlfammatory and wound healing enhancement properties of honey. More recent studies have shown that the addition of wound honey to topical antibiotics improves their effectiveness. This is not your usual cooking or table honey that you put on pancakes. Medihoney and manuka honey are the main wound honeys. They are bitter and not fit for consumption as a food item.

DRESSINGS IMPREGNATED WITH MEDICATIONS

  • Silver Impregnated and Honey Impregnated Alginate Dressings
  • Medihoney impreganted alginate

DRESSING SELECTION

Before employing any of these medications or dressings you should be cleared by your personal physician and have your ongoing care including frequency of dressing changes and bath soaks supervised by them to avoid sensitivity or allergic reactions that can present immediately or after multiple applications of an initially well tolerated product. Even the adhesive in a bandage or on a tape can be problematic. Obviously if you are allergic to bee stings or honey medihoney is not a viable option.

If you are pre-stage I resorcinol or medihoney under a transparent dressing is a good option as they will reduce the inflammation while allowing you to directly observe the area for disease progression.

If you are stage I with visible abscess infections but no drainage clindamycin ointment and/or medihoney under a transparent dressing is a good option. This should treat the infection while allowing direct observation of the area. The ointment cannot be used for prolonged periods because you will end up with resistant bacteria.

Once you start having drainage alginate or foam dressings are better options to soak up this drainage. Foam is better when the wounds have depth to them as foam is thicker than alginate allowing more wound packing with less dressing. Either one can be obtained with silver or medihoney impregnated to fight the infection at the same time as the drainage is collected. They come in sizes as large as 6x6cm. The impregnated forms are quite expensive while tubes of medihoney can bought on Amazon for $15 a tube and applied to the cheaper non-impregnated forms prior to application.

As the disease process subsides you can move on to mesalt dressings and if there are open areas without drainage hydrocolloid dressings are very good options. Hydrocolloid dressings applied to raw areas after surgical excision of hidradenitis have been proven to minimize discomfort, allow faster mobilization, remain dryer longer and reduce nursing time compared to conventional gauze dressings. One of my patients had a raw spot on each buttock after surgery whose edges would stick together when sleeping and tear apart when sitting or walking. The areas could therefore not heal. Application of Duoderm to each spot stopped them from sticking together at night and they healed over.

KEEPING DRESSINGS IN PLACE

Taping dressings in place can be problematic if the drainage has an odor or large volume as the frequent dressing changes, removing the tape each time, chafes the surrounding skin. These are 2 good options to keep the dressings in place.

–Tubular Elastic Net Dressing

You can create shorts made of tubular elastic to hold dressings on the groin or buttocks.

–Montgomery Straps

The outer white parts of Montgomery Straps stick to the skin on either side of the wound and laces are threaded between the holes to keep a dressing on/in a wound. You untie and tie the laces to change the dressing that way you don’t have to keep ripping tape off of your skin. This is especially good for wounds that drain a lot of fluid. You can put an alginate in there and hold it in place with the laces. The alginate will soak up the drainage like a sponge. You can even change the dressing 3 or 4 times a day if you need to.

You can make your own by using 3 inch wide medical tape and folding the adhesive side onto itself along one edge. Punch holes in the double thickness portion of the tape to thread laces through and you have made your own montgomery strap.
