It is important to note advance bookings must be made at Namibia Wildlife Resorts main office in Windhoek or online. Reservations for Etosha camps cannot be made over the telephone, and should be made well in advance.

How to Break in Prescription Orthotic Shoe Inserts

The use of true prescription-level orthotics to aid in the treatment of foot ailments has steadily increased over the last thirty years. Commonly prescribed by podiatrists, sports medicine doctors, and other medical professionals, orthotics function to provide support for flat arches, shock absorption for high arches, and allow for an excessive amount of control to the pressures of the heel and the bottom of the foot. Because each orthotic is made specifically from a structurally corrected mold of the foot, the alteration and improvement of foot function is dramatically improved over that of less specific over-the-counter inserts and inserts made of a non-corrected mold of the foot. Each orthotic should 'fit like a glove', so to speak, and should be completely satisfied to wear the entire day. Most people can begin wearing new orthotics without discomfort right from the start. However, there are a small amount of people who require a break-in period to get the feet used to standing on the inserts all day. This article discusses this break-in process, and how one can get the most out of their new orthotics in as little time as possible.

When new orthotics are uncomfortable to wear, it can be incredibly frustrating. Many people expect instant relief of foot discomfort, and this is understandable given the nature of foot pain and the costs sometimes contributed to get orthotics in the first place. Orhotics should not cause discomfort. One has to assume that they were properly casted for in the first place, and the proper prescription was used to make additional configurations to the basic design based on the physician's examination of the foot structure. One also has to assume the lab correctly fabricated the insert to the proper specifications. To immediately assume that either one of these steps were done incorrectly does not help what really must be done: a break-in period performed with patience and persistence. The frequency of improper casting, prescription writing, or manufacture of orthotics is usually quite low. These factors should only be realistically considered after all other measures have failed to provide last comfort.

The break-in period is a short span of time in which the foot becomes accustomed to the control that the orthotic is providing. In essence, a proper orthotic will provide gentle but firm support of the foot structures. This includes support of all the muscles and ligaments which have become lax during a lifetime of abnormal support. The orthotic will force these structures to realign properly. For some, this can cause a brief irritation of the lax tissue, or it can force some muscles to function properly for the first time. These actions can cause discomfort, and this will be felt while the orthotic is worn. A simple plan to gently introduce the foot to increased support is vital in making the orthotic comfortable to wear. When one first receives their orthotics, they should be worn immediately. When and if the orthotics become uncomfortable, they should be removed from the shoes. After several hours, they can be placed back in the shoe and used again until discomfort develops. Alternately, one can wait until the next day to try again. This cycle is to be repeated daily, with a longer wear time achieved each day. Occasionally, after two or three weeks, the orthotics will no longer be uncomfortable. Another common strategy is to wear the orthotics for one hour the first day, and then increase the wear time by one hour each day that follows. In this author's professional experience, it is very uncommon for a properly followed break-in period to fail providing a complete adjustment of the foot to the orthotic.

There are other factors that do contribute to the failure of a break-in period leading to comfort with an orthotic. The most common of these involves an incompatibility of the shoe with the orthotic. Although orthotics are designed to fit into a wide variety of shoes, not every shoe in circulation is appropriate for an orthotic. The orthotic should not slide under the foot with shoe use. This indicates a shoe that is sized to big for one's actual foot size. They also should not have to be crammed into the shoe, with little room left for the foot. This indicates the shoe is too small for the foot or is too small or narrow to work with an orthotic. Stubbornly holding on to improper shoes at the expense of orthotic comfort strictly for style reasons is foolish, and will never lead to a comfortable orthotic. One must be sensible and reasonable regarding their shoe selection. This does not mean a sacrifice of fashion sensitivity, it simply means being reasonable and accepting of ones own body and foot structure needs. Another good example of this is the high heel. High heels force the body to place great pressure on the ball of the foot. This takes away the natural walking pattern the body uses to propel itself. An orthotic will never function properly in a high heel shoe as the body does not use the foot properly during this abnormal walking pattern, and pressure is not properly transferred through the orthotic. Although some boutique-style orthotics are constructed to be used in these types of shoes, their usefulness is debatable. Another factor contributing to break-in failure involves instances in which a shoe may properly fit, but certain design choices by the manufacturer creates a limitation for proper orthotic use. For example, some athletic shoes contain extra padding around the top of where the shoe hits the heel.

This padding pushes the heel slightly forward in the shoe, causing the foot to slide too far down the orthotic. The 'arch' of the orthotic will now be slightly further back, and this can be the source of discomfort. Another example includes shoes that already add arch padding inside the shoe. Although this padding on its own offers little true support, the bulk of its volume can force the orthotic slightly out of alignment in the shoe, or it may prevent its fitting altogether. This internal padding needs to be removed prior to wearing orthotics. Sometimes this material is easily removed in one loose piece, and other times it is lightly glued into the shoe. If it is stitched in, removal will be difficult and may leave an rough surface for the foot to rest on. A different shoe should be selected in this case. Finally, one must consider the medical condition leading to the use of orthotics in the first place as a possible cause for break-in failure. When someone has a foot condition in which there is a constant state of inflammation and tissue damage, and this has not yet healed, then the introduction of a firm device under the irritated tissue can cause pain and discomfort. Certain conditions, such as those involving heel or arch damage, must be healed or adequately improved before an orthotic can be tolerated properly. Once the condition is treated, then the orthotic will function more comfortably and will usually contribute to the prevention of the condition returning if it was related to one of foot structure in the first place (as many foot problems are).

Once all outside factors are accounted for, and one has gone although a proper break-in period, any continuation of orthotic discomfort should be a cause for concern. It is at this very uncommon time that one must consider factors like improper casting, prescribing, and manufacture as a cause for continued discomfort. The physician prescribing the orthotic should review the orthotic completely at this point, as subtle changes or problems can sometimes be missed that can contribute to an uncomfortable orthotic. At times the orthotic may need to be returned to the lab for modification or replacement. In many instances, a simple modification in the doctor's office can bring about an alteration that is successful in making the orthotics comfortable. With all this in mind, one final factor needs to be considered. There are those out there, who by personal quirk, psychological irritation, or bad luck simply can not tolerate anything to be placed under the feet outside of a loose and cushioned shoe. Unfortunately, these people will never be able to tolerate an orthotic. Fortunately, this is an extremely rare occurrence in this author's experience.

Basic Greenhouse Designs

Constructing a greenhouse requires you to finalize a design or style. A variety of greenhouse designs are available to choose from. You can take your pick according to your requirement, style, taste and budget. Greenhouse designs are broadly categorized into three types: attached, ridge or furrow, and detached.

Attached greenhouses

Lean-to greenhouse:  When space is limited and you want to save up on costs, an attached greenhouse is a good option. The lean-to greenhouse has a ridge line that is attached to one wall of your home. Usually, a connecting door provides direct access from your home to the interior of the greenhouse. Proximity with the main structure of your dwelling provides easy access to power sockets and water source. The lean-to comes with straight or curved eave design. The roof is always sloping to allow snow, rain and leaves to slide off.

Window greenhouse: The smallest of all the attached greenhouses are the window mounted variety with just two or three shelves that can accommodate a few of your favorite plants.

Even-span greenhouse: It can be put up as an attached or a free-standing structure. In the attached variety, one gable end is attached to the end wall of another building. It is the largest of the garden greenhouses and has the advantage of allowing you to lengthen it if you start on a budget and later decide to upgrade and upscale the structure.  It scores over the lean-to in having a design that is more efficient as far as heating, cooling, ventilation and optimum use of available space are concerned.

Ridge or furrow greenhouses 

These are generally free-standing structures with gabled or arched rooflines. These are attached only at a common gutter at the lower edges of the eaves of the roof with no division by walls of the structure under the canopy. The curved arches are preferred if you are planning to use flexible glazing material like polyethylene. The gabled roof can have more rigid material like glass or fiberglass.  Being relatively large, this type of greenhouse is usually built by commercial growers.

Detached greenhouses

These are independent or stand-alone free standing structures situated away from the home. They have the advantage of being larger and admit more sunshine as there are no shadows of an attached structure to hamper sunlight.

Quonset greenhouse or hoop house is a popular option because it is quite inexpensive and easy to put up. It is arched and uses light weight framing over which flexible glazing can be stretched.

The Gothic arch greenhousehas great aesthetic appeal. It consists of two separate curved pieces that meet at the ridge line of the roof. As the curvature is less pronounced than the Quonset, it provides more head room and usable space.

Classic A-Frames look like the alphabet ‘A’ when put up. They are easily assembled on the ground before being erected, but have relatively less usable space.

Modified A-Frame greenhouses are shaped typically like a house. With straight vertical walls topped by gabled roof without eaves, they have more usable space than the classic A-Frames.

Barn style greenhouse has greater usable space as the peaked roof slants to meet eaves on either side of the ridge line. The eaves at the roof’s edge connect it to the sidewalls. The sidewalls may be straight or slanted; the latter providing for greater sunny span.

Dome greenhouses are semi-circular. They are great for minimizing wind resistance and allowing maximum light transmission.

Modern Packaged Boiler Operation, Maintenance and Training

The latter part of the 20th Century saw the introduction of more stringent boiler manufacturing standards, particularly in relation to welding technology and inspection, and the evolution of digital control systems that now provides an unsurpassed level of reliability. The modern, micro-processor based, high-integrity, "Boiler Control System" (BCS) has the ability to perform many of the tasks traditionally admitted by the qualified operator allowing extended periods of automatic operation without any human intervention whateversoever. A key feature of modern packaged boilers is that they are designed to operate unsupervised for extended periods.

The degree of human supervision and operator training requirements are determined by the level of sophistication of the "Boiler Control System" (BCS). They are defined in local boiler codes and standards and differ widely between jurisdictions. For example, in some, a high-pressure boiler is defined as any boiler operating above 1 BAR (approximately 15 psig) pressure and, regardless of all other considerations, it must at all times be under the control of an appropriately qualified operator. Other jurisprudence imposes capacity constraints on unsupervised operation but in many, boiler codes and other relevant legislation have been revised and extended to accommodate the demand from industry for this type of boiler.

Industrial boilers of all types generally fall into one of three categories in terms of their operational and qualification requirements. They are:

• Attended Boilers;
• Limited-Attendance Boilers;
• Unattended Boilers.

What is a "packaged boiler"?

The term "Packaged Boiler" is used to describe a boiler that is factory built and delivered either fully assembled or, as pre-built modules that can be easily assembled on site. It is essentially a self contained unit complete with a control system and all equipment required for operation. The boiler manufacturer assumes responsibility for all components and after delivery all that is required is for it to be connected to the steam distribution system, water, fuel, and electricity supplies. Almost all small and medium sized boilers manufactured today are packaged boilers and they may be the conventional firetube or watertube type, including their derivatives or, hybrid boilers such as once-through coil and electrode boilers.

What is an attended boiler?

Until relatively recently, the vast majority of steam boilers operated in the "Attended" mode. That is, they were required to have an appropriately qualified operator on-site and in the immediate vicity of the boiler at all times that it was operating. Since the introduction of micro-processor based technology to boilers from around 1990 onwards, many traditional attended boiler owners have upgraded their control systems. Given also that micro-processor based controls are now an almost universal feature on new packaged-boilers, the traditional boiler attendant is becoming an endangered species in small to medium sized plants supplying process steam. At the very least, their role has changed from that of direct, hands-on operation to one of supervising the operation of the BCS and performing tasks that ensure its continuing integrity and compliance with the requirements of boiler codes and standards.

Boiler operation has evolved to the point that in many jurisdictions, fully attended operation is unusual outside of large, complex plants. The exceptions are large power utilities, chemical recovery boilers and similar integrated processes where the boiler is a key component.

what is a limited-attendance boiler?

The Limited-Attendance mode of operation allows for periodic checking of the boiler at set minimum intervals rather than continuous attendance. A key feature of limited-attendance boilers is that they may at any time be brought under the control of an appropriately qualified operator and operated in the fully attended mode under continuous supervision.

Many older packaged boilers and those that have been upgraded from attended status are classified as limited-attendance simply because their control systems do not achieve the same standards of integrity and reliability as do modern micro-processor based systems. Many also include provision for manual intervention which in most jurisprudence disqualifies them for consideration for unattended status.

What is an unattended boiler?

An Unattended Boiler is essentially a fully automatic boiler that operates under the continuous supervision and control of a standardized, micro-processor based, BCS. The BCS controls the entire boiler function including the energy input management system (on oil and gas-fired boilers commonly known as the "Burner Management System" or, BMS), the water level management system, the alarm system, the pressure controls, trip devices, all instruments and circuitry.

A key feature of this type of boiler is that in most jurisdictions boiler codes and standards dictate that a boiler that is classified as unattended must be under the continuous supervision and control of the BCS at all times that it is in service. The unattended boiler may only be operated in unattended mode. It may not be operated in attended mode under any circumstances even for brief periods and even if a qualified operator is available. In practice, the BCS employed an unattended boiler has no provision for manual operation. In the event of a control system fault or a potentially unsafe condition, the BCS will shut down the boiler and lock-out the energy input system. It will prevent a boiler restart until such time that the control system fault has been repaired or, the potentially unsafe condition has been addressed and all of the BCS inputs indicate that the boiler is in a safe condition.

The BCS is equipped with some means of recording all major events that cause the alarms to activate. On early unattended boilers this was typically a simple printer that recorded such events on a line-by-line basis. More modern systems however, store the information in the onboard micro-processor memory for an extended period. They typically record a much wider range of information to provide improved oversight of boiler performance and, are sometimes integrated with a plant-wide information system which facilitates the remote storage of data for extended periods.

The unattended boiler is a relatively recent phenomenon. Traditionally most boiler installations operated under the continuous supervision of a qualified operator although in many jurisdictions, some small boilers were permitted to operate in limited-attendance mode. Until very recently, in most jurisprudence unattended boiler operation was subject to strict constraints. Typically, permitted maximum operating pressure and design output were strictly limited and in many locations those constraints still apply, at least on paper. In recent years however, boiler codes and standards in many jurisdictions have been extended to accommodate this type of boiler and in others it has become quite common for local regulatory authorities to grant exemptions on a case-by-case basis.

With the advent in recent years of improved manufacturing standards and the rapid evolution of sophisticated, increasingly reliable micro-processor based control and monitoring systems, most small to medium sized packaged boilers from reputable manufacturers now incorporate a high-integrity BCS as standard equipment. Where unattended operation is permitted, they may only require inspection and operational certification by an accredited inspection body to ensure compliance with national and local standards.

Limited-Attendance vs Unattended Operation

To achieve Limited-Attendance or Unattended classification, the boiler is subject to strict statutory oversight covering operation, maintenance, servicing and record keeping. Boiler codes and standards require that boilers in either category be equipped with a minimum, specified inventory of safety equipment that will automatically shut down the boiler in the event of a potentially unsafe condition.

The classification applied to a particular packaged boiler will depend heavily on the sophistication of the BCS or, sometimes, the type of fuel that is fired. For example, a 10 MW boiler firing natural gas may achieve unattended classification where a similar 10 MW boiler from the same manufacturer but firing solid-fuel may be classified as limited-attendance. Firing solid-fuels, particularly those with a variable moisture content, is sometimes something of a black-art requiring a degree of skill and experience. In this author's jurisdiction, unattended solid-fuel boilers are abruptless not uncommon even for quite large boilers over 20 MW capacity and often in multiple boiler installations.

The key differences between the Limited-Attendance and Unattended modes of operation are:

Limited Attendance

• An appropriately qualified operator or liable person must be on-site at all times that the boiler is in service;

• Codes and standards typically require that the boiler be continuously supervised during startup and shutdown periods;

• During normal operation, the limited-attendance mode allows for periodic checking of the boiler at set minimum intervals rather than continuous supervision;

• Limited-Attendance boilers may at any time be brought under the control of an appropriately qualified operator and operated in the fully attended mode under continuous supervision;

• The operator or responsible person must be on-site at all times that the boiler is in service and must be immediately available if required.

Unattended

• Where unattended operation is permitted, a qualified operator is not a mandatory requirement. The boiler may be supervised by an appropriately trained "Responsible person";

• The operator or responsible person may leave the site provided that he or she is available to respond to boiler outputs. That is usually achieved by a pager or cellphone signal transmitted by the BCS;

• In many jurisdictions, unattended boilers may start up and shut down automatically under the control of the BCS provided that they are suitably equipped to the satisfaction of an approved inspection body;

• Unattended boilers are allowed to operate unsupervised for extended periods and in most jurisdictions this is currently 24-hours. Recent control and monitoring systems innovations by some manufacturers however, have seen that period extended in some jurisdictions. The 72H notation in Europe for example, signifies that the boiler is approved for unsupervised operation for periods of up to 72-hours;

• Boiler codes in most jurisdictions specifically forbid the provision for manual intervention in the operation of unattended boilers. They must at all times be under the control of the BCS. That is, they may only be operated in unattended mode. They may not be operated in attended mode under any circumstances even for brief periods and even if a qualified operator is available.

Training Requirements

Boilers that are certified for unattended operation do not require a qualified operator. This relaxation of the manning requirements imposes substantive obligations on boiler owners, controllers, manufacturers and inspection bodies to ensure compliance with boiler codes and other relevant legislation and, that boiler supervision and maintenance staff receive adequate training. Non compliance will almost always be interpreted as a breach of Occupational Safety and Health legislation which can result in severe penalties specifically because the non-compliance is discovered as a result of an incident or accident.

Accident investigation reports demonstrate that the most common contributing causes to boiler accidents are operator intervention with the safety controls, inadequate maintenance, control device failure following maintenance by unqualified personnel, isolation or deliberate disablement of the safety devices, inappropriate operating practices and inadequate water treatment. Automatic controls are not a complete substitute for supervision. A measure of supervision by a qualified operator or a trained Responsible person familiar with the BCS and the operation of the boiler plant is an essential feature of safe operation.

Unattended boilers that are operated in strict compliance with the manufacturer's recommendations, boiler codes and standards have an excellent safety record worldwide. Because they are designed to operate unsupervised for extended periods, routine operating, maintenance and inspection requirements are generally more stringent than for other types of boilers. The BCS and its associated safety devices clearly play a key role in the safe operation of this type of boiler and are there subject to strict compliance with routine testing procedures by the trained Responsible person and the approved maintenance contractor. It is extremely important that there should be a high level of confidence in the integrity of the BCS and the routine testing procedures that should be considered as a key element of safe operation.

Although those tests are generally universal in nature, there are differences in their implementation between jurisdictions. It is there before very important that the boiler owner, controller and trained Responsible person understand that they have a responsibility to familiarize themselves with the local codes and regulations applying to this type of boiler. Failure to meet that obligation will usually be interpreted as a breach of OSHA regulations. If in doubt, you should seek clarification from your boiler inspector, inspection body, the boiler manufacturer or the local regulatory authority.

Who is the boiler controller? – roles and responsibilities:

Boiler codes and standards generally require that unattended boiler installations have a nominated "Controller". This may be the owner or, a person nominated by the owner to perform that role. The Controller does not necessarily have to be closely involved with hands-on, day-to-day operation but rather, to provide general oversight of the boiler operation and maintenance.

The Controller's responsibilities include:

• Ensuring compliance with the Boiler code or relevant standard including the maintenance of appropriate operational and engineering records;

• Ensuring that the boiler is supervised by appropriately qualified or trained persons;

• The implementation and maintenance of a Quality Management System (QMS);

• The implementation and maintenance of appropriate safety and environmental procedures and, communications with the appropriate regulatory bodies for example, OSHA, environmental agencies, inspection bodies and so on.

The Controller is typically a engineering, maintenance or departmental manager or, someone in the organization that holds a qualification or Certificate of Competency in boiler operation.

what is a trained responsible person? – roles and responsibilities:

A "Trained Responsible Person" is someone who is appointed by the controller to exercise day-to-day supervision of the boiler. He or she is not required to hold a formal qualification such as a certificate of competency but, must be trained to a level specified by the boiler manufacturer or, to a level acceptable by an inspection body or other appropriate regulatory authority. The Responsible person is not required to have an intimate knowledge of boiler technology but rather, a basic level of knowledge and an understanding of the functionality and roles of the boiler components, mountings and ancillary systems including the fuel, feedwater, and steam distribution systems.

The primary roles of the Responsible person are to exercise general day-to-day supervision of the boiler operation, verify that all control and alarm systems are functioning correctly and to ensure compliance with the routine testing procedures specified in the relevant code or standard and, by the boiler manufacturer. The Responsible person should not necessarily be expected to diagnose and rectify equipment or system failure. They should be competent to respond to alerts, and to take appropriate action which, at a minimum, ensures that the boiler is shut down safely before calling for assistance.

what is an approved maintenance contractor ?:

Compliance with defined maintenance procedures is a key aspect of unattended boiler operation. There have been a number of recent catastrophic boiler accidents resulting from inappropriately performed maintenance tasks which in some cases, were undertaken by qualified and experienced tradesmen but, who lacked the specialist knowledge requirements associated with boiler maintenance.

The "Approved Maintenance Contractor" is a person or organization with the appropriate skills, knowledge and experience to maintain the boiler and its components to a standard acceptable by the manufacturer and the inspection body. They must have a thorough understanding of the role and functionality of the BCS in relation to limited-attendance and unattended operation.

Boiler codes and regulations define minimum acceptable standards for this role which may differ between jurisdictions. In general, they will require that the nominated organization be a boiler manufacturer or an independent contractor approved by the manufacturer or the inspection body. In organizations that have their own internal maintenance infrastructure, the Controller may apply to the inspection body to nominate the organization itself as the approved maintenance entity. In such cases, the organization must satisfy the requirement that nominated personnel performing the work have appropriate training and experience. That will typically require that nominated staff members can provide evidence of an appropriate trade qualification or, have received specialist training and have sufficient experience. Under no circumstances whatever should not-approved staff be allowed to perform maintenance on any pressure equipment or the BCS and its field devices including all safety devices and the energy input system.
